Nitroglycerin Injection

Nitroglycerin injection is an intravenous vasodilator that acts predominantly on venous smooth muscle to reduce preload, and at higher doses, reduces arterial resistance (afterload). It is rapidly metabolized and requires continuous blood pressure monitoring.

Dosage by Body Weight

Administer only via continuous intravenous infusion using an infusion pump. Dilution in 5% Dextrose or 0.9% Sodium Chloride is required. Start at 5 mcg/min and titrate based on hemodynamic response. Do not administer by direct IV bolus.

  1. Body Weight
    Below 5 kg
    Dose
    0.25 to 1.0 mcg/kg/min
    Frequency
    Continuous IV infusion
    Tracking Notes
Requires intensive care monitoring. Titrate slowly to avoid severe hypotension.
  2. Body Weight
    5-7 kg
    Dose
    1.25 to 7.0 mcg/min
    Frequency
    Continuous IV infusion
    Tracking Notes
Calculate exact dose based on patient's weight in kg and clinical response.
  3. Body Weight
    8-10 kg
    Dose
    2.0 to 10.0 mcg/min
    Frequency
    Continuous IV infusion
    Tracking Notes
Monitor arterial blood pressure and heart rate continuously.
  4. Body Weight
    11-15 kg
    Dose
    2.75 to 15.0 mcg/min
    Frequency
    Continuous IV infusion
    Tracking Notes
Adjust infusion rate in small increments every 3 to 5 minutes.
  5. Body Weight
    16-20 kg
    Dose
    4.0 to 20.0 mcg/min
    Frequency
    Continuous IV infusion
    Tracking Notes
Use non-PVC infusion sets as nitroglycerin migrates into standard PVC plastics.
