Vitamin K antagonist. Requires very frequent monitoring of INR (blood clotting time) and is heavily influenced by diet (green leafy vegetables).

Dosage by Body Weight

Pediatric starting dose: 0.1 mg/kg to 0.2 mg/kg once daily. Maintenance dose is highly variable.
