Sotalol

Sotalol is a dual-action cardiovascular agent possessing both Class II (beta-adrenergic blocking) and Class III (cardiac action potential duration prolonging) antiarrhythmic properties. Because it can cause life-threatening ventricular arrhythmias associated with QT prolongation, sotalol therapy must be initiated or up-titrated in an inpatient setting equipped with continuous ECG monitoring and cardiac resuscitation facilities.

Dosage by Body Weight

Adult dosing usually starts at 80 mg orally twice daily. Dosage adjustments are made gradually every 3 days based on clinical response, heart rate, QT interval, and renal function (creatinine clearance).

  1. Body Weight
    Below 5 kg
    Dose
    Strictly individualized
    Frequency
2 to 3 times daily as prescribed
    Tracking Notes
Must be calculated using pediatric Body Surface Area (BSA) formulas with age-appropriate down-titration (for infants under 2 years) under inpatient pediatric cardiology supervision.
  2. Body Weight
    5-7 kg
    Dose
    Strictly individualized
    Frequency
2 to 3 times daily as prescribed
    Tracking Notes
Dose is strictly calculated based on Body Surface Area (BSA) and renal clearance. Requires in-hospital ECG monitoring during initiation.
  3. Body Weight
    8-10 kg
    Dose
    Strictly individualized
    Frequency
2 to 3 times daily as prescribed
    Tracking Notes
Requires pediatric oral solution formulations for highly accurate dosing.
  4. Body Weight
    11-15 kg
    Dose
    Strictly individualized
    Frequency
2 to 3 times daily as prescribed
    Tracking Notes
Dosing frequency and amount are tailored dynamically according to QTc interval and renal function.
  5. Body Weight
    16-20 kg
    Dose
    Strictly individualized
    Frequency
2 to 3 times daily as prescribed
    Tracking Notes
Dose must be converted from adult standards using established pediatric Nomograms (BSA-based scaling).
