Enoxaparin (Pediatric)

A Low Molecular Weight Heparin (LMWH). Dosing in infants is significantly higher than in adults to achieve the same anticoagulant effect.

Dosage by Body Weight

Treatment dose (Infants <2 months): 1.5 mg/kg every 12 hours. Children >2 months: 1 mg/kg every 12 hours. Monitor Anti-Xa levels.
