Clexane

Clexane contains enoxaparin sodium, which is a low-molecular-weight heparin (LMWH). It works by inhibiting clotting factors (mainly Factor Xa) in the blood to prevent the formation of blood clots and to stop existing clots from expanding, allowing the body to naturally break them down.

Dosage by Body Weight

Administered via subcutaneous injection (or intravenously in acute cardiac settings). Dosage is highly individualized based on body weight, medical indication, and renal function. Pediatric dosing must be calculated strictly on a mg/kg basis and monitored by a specialist.

  1. Body Weight
    Below 5 kg
    Dose
Requires exact weight-based calculation (typically 1.5 mg/kg for neonates, 1 mg/kg for older infants)
    Frequency
    Every 12 hours
    Tracking Notes
Strict monitoring of anti-Factor Xa levels is essential. Do not use standard prefilled syringe graduations.
  2. Body Weight
    5-7 kg
    Dose
    5 mg to 7 mg (calculated at 1 mg/kg)
    Frequency
    Every 12 hours
    Tracking Notes
Use a low-volume syringe for precise measurement. Rotate injection sites daily.
  3. Body Weight
    8-10 kg
    Dose
    8 mg to 10 mg (calculated at 1 mg/kg)
    Frequency
    Every 12 hours
    Tracking Notes
Adjust dosage downwards if severe renal impairment is present.
  4. Body Weight
    11-15 kg
    Dose
    11 mg to 15 mg (calculated at 1 mg/kg)
    Frequency
    Every 12 hours
    Tracking Notes
Prophylactic dosing is lower, typically 0.5 mg/kg every 12 to 24 hours.
  5. Body Weight
    16-20 kg
    Dose
    16 mg to 20 mg (calculated at 1 mg/kg)
    Frequency
    Every 12 hours
    Tracking Notes
Monitor for any unusual bruising, bleeding, or pain at the injection site.
