Vancomycin (Systemic)
A glycopeptide antibiotic. Systemic use (IV) is reserved for severe infections like endocarditis or bone infections. It requires drug level monitoring (Trough levels) to avoid kidney toxicity.
Dosage by Body Weight
Pediatric dose: 15 mg/kg every 6 to 8 hours. Infusion must be slow (at least 60 mins) to prevent 'Red Man Syndrome'.
