Empagliflozin

Empagliflozin is an inhibitor of the sodium-glucose co-transporter 2 (SGLT2), the primary transporter responsible for renal glucose reabsorption. By inhibiting SGLT2, empagliflozin reduces renal reabsorption of filtered glucose and lowers the renal threshold for glucose, thereby increasing urinary glucose excretion and lowering blood glucose levels.

Dosage by Body Weight

The recommended starting dose is 10 mg orally once daily in the morning, taken with or without food. In patients tolerating 10 mg daily who require additional glycemic control, the dose may be increased to 25 mg once daily. Renal function (eGFR) must be assessed prior to initiation and periodically thereafter.

Purpose

Empagliflozin is indicated as an adjunct to diet and exercise to improve glycemic control in adults and pediatric patients aged 10 years and older with type 2 diabetes mellitus. It is also used to reduce the risk of cardiovascular death in adults with type 2 diabetes and established cardiovascular disease, and to reduce the risk of cardiovascular death and hospitalization in adults with heart failure.
